Recently, the clinical research result “Randomized Trial of Self-Adaptive Computerized Cognitive Remediation in Schizophrenia” was published in the top international academic journal “NPJ Digital Medicine” ( Impact factor 18.0, JCR Q1 zone).
The study was a randomized, double-blind, actively controlled clinical trial (ChicTR2000040326). A total of 90 patients with schizophrenia were randomly divided into an adaptive difficulty training group (SAD, 41 completed intervention) and a fixed difficulty training group (FD, 31 completed intervention) according to 1:1 to compare the cognitive and clinical outcomes of the two groups during 4 weeks of intervention and 8 weeks of follow-up. Both groups of subjects completed the training using the Group's “Brain Function Information Management Platform Software System” (Class II medical device). Cognitive outcomes were assessed using the Cognitive Index (CI) and the MATRICS Consensus Cognitive Suite (MCCB).
Main results: (1) Faster and stronger cognitive improvement: The linear mixed effect model showed that the adaptive group achieved significantly greater and faster improvements (P=0.02) in terms of attention and processing speed/agility, and a faster improvement trend was also observed in terms of thinking. (2) Significant improvement in overall MCCB score: The adaptive group's continuous operation test (CPT-IP) and total MCCB score were significantly superior to that of the control group. (3) Significant improvement in positive symptoms: At the end of 4 weeks of training, the positive symptom scores on the adaptive group's positive and negative symptom scales decreased significantly compared to the control group. (4) The mechanism of “difficulty - performance coupling” was revealed for the first time: the study observed a steady difficult-performance coupling effect in the five cognitive domains of memory, attention, perception, agility, and thinking, indicating that dynamic difficulty calibration is a key active mechanism driving cognitive benefit.
The study concluded that adaptive cognitive training can enhance and accelerate cognitive improvement in patients with schizophrenia, particularly in terms of attention and processing speed, and is also beneficial to improving positive symptoms; the difficulty - performance coupling may reflect the role of dynamic load calibration as a key treatment mechanism for digital cognitive training.
